Proof of Immunization or Waiver Process

All ULM students (including those completely online) are required to show proof of all required immunizations OR have an exemption recorded. Follow these steps to submit your completed immunization record or exemption form:

Step 1: Scan or take a picture of your completed immunization record or exemption form.

Step 2: Find your username and password.

  1. Visit
  2. Enter your Campus-Wide ID
  3. Make note of your username
  4. Initial password is your birth month, birth day, and year, followed by a dash and the last 4 digits of your CWID. Ex: 090307-1234

Step 3: Upload the scan or photo of your immunization record of exemption form.

  1. Visit
  2. Enter your username and password
  3. Click “upload” near the bottom
  4. Choose immunization record or immunization exemption form
  5. Click select file to upload picture.

Students who do not submit the required immunization information or exemption form will have an immunization hold preventing them for registering for classes. Students needing assistance can emailadmissions@ulm.edu.
